Another Free printable I created just for you! This one is a reference sheet for your babysitter. You can create copies and give them to your sitters to take home and you can laminate one and keep it on the fridge for easy reference.
The first section is important. You'd be surprised how details escape a person's mind in emergencies, make sure that your full address and land line phone number are filled out on the form. If the babysitter has to call 911, the information will be right at hand to give to the operator.
Fill in the rest of the information so your sitter can easily contact you with questions and has a quick reference to remember important items like allergies and medical conditions. Print more than one or attach additional sheets for each child.
So download the forms, copy them, pass them out, link to them, whatever you want. Thanks!
